In compliance with the POPI Act, no 4 of 2013, which came into effect on 30 June 2020,  we need to add a paragraph at the beginning of our Data Verification Form in which we inform the parents why we need their details and what we do with their details. 

If you remove the following irrelevant sections from page 1 of the data verification form, there will be enough space to place the POPIA paragraph.

 

·        Pre-Primary education attended.

·        Registered for social grant

·        Receives social grant

·        Benefit from School nutrition programme

·        Learner resides in a hostel

·        Name of hostel

·        Method of transport

·        Vehicle registration / route

·        Name of driver

·        Contact number

·        The entire section regarding “Information of previous school/play group/nursery”

 

Please also change the little paragraph at the top of the page to: Please check and correct information. Sign in pen in all places.
