Good morning.  I think it would be beneficial to all schools if you create a new standard report with an indemnity form.  It can look almost the same as the data verifications report that is already under learner reports.  

Just a bit of background as to why we need this report....  Seeing that as a school our learners participate in many excursions, we need parents to sign an  indemnity form at the beginning of the year.  This indemnity form is then kept by the different register teachers or Head of departments.  When an excursion occurs the relevant coach/teacher ensures that he/she has all the participants indemnity forms on hand. 

If we can have such a report on D6 we can pull the information per class and D6 can populate the report with each learners personal data, it can be sent to the parents for signatures.  Now we do not have a standard report and the parents have to fill all the details in themselves and then sign. 

It can also serve as a data verification tool, if there is some personal information that needs to change, we will be informed and make the relevant changes.
