The form loaded in D6 to use for leave is the Z1 form which is the correct form for Non Educator State Paid Personell . 

The correct form for Educators is the H1 form which is on page I - 198 of the PAM Document 

It is very confusing having the wrong form loaded in the system please could you include the H1 form for us to use for Educators as we can only get horrible copies off the internet currently
