As a school we deal with injuries daily and incidents as well 

i would recommend but the serious incidents to add a couple of extra futures like adding who was it reported to which teacher, and did the teacher reported it to the grade head, and the after the grade head there has to be a drop down on with disciplinary head must sort it out.

and the under the type of incident you can have to choose like fighting, clapping in face and swearing at the teacher or learner...
