Registration Forms

The Registration Forms function prints a blank registration form for new learners. Some sections of the form can be customised according to the school's preference.

How do I print Registration Forms?

Select the following options and click View to generate the forms:

  • For year: Select the year to appear in the heading of the form.

  • Print for paper: Select A4 or A3.

  • Form heading: Select the relevant heading.

  • Undertaking of biological parent: Indicate whether this section should appear on the form.

  • Print responsible person/Debit order information: Indicate whether these details should be included (applicable if the school uses d6 Finance).

  • Debit order payments over: Select 10, 11 or 12 months.

  • Print contract with school: Indicate whether the contract and consent section should be included.

  • Payments over: Select 10, 11 or 12 months.

How do I edit the contract on the form?

The following sections of the form can be edited: Contract, Indemnity/Consent, Indemnity, Parent undertaking and Debit order authorisation. Use the relevant tab to edit each section. Click Add to add additional lines. Each line has fields for Indent level, Paragraph number, Option choice, Content (Eng/Afr), Next page, Heading, Sort and Status (Active, Inactive or Deleted).

Frequently asked questions

Can I import learner information electronically?

Yes. It is possible to import learner information using an Excel template. Schools can create a Google form to be completed by parents. See the instructions and example in the Learners article in the knowledge base.

What do the Status options for a line mean?

Active — the line appears on the form. Inactive — the line does not currently appear but can be reactivated. Deleted — the line is permanently deleted and cannot be recovered.

Can I print the form in both English and Afrikaans?

Yes. Enter the content of each line in both the Content Eng and Content Afr fields. The appropriate language version is printed depending on the language selection during the print process.
