How to generate a leave application form

  1. Choose between the H.1 or Z1(a) form depending on the staff member's role.

  2. Click the Preview button to generate the Application for Leave of Absence form.

  3. The form is generated as a PDF document that can be printed.

  4. Print the form and have the staff member complete it before submitting for approval.
